About the Role

As the Clinical Manager, you'll oversee the delivery of high-quality, patient-centered care in a home setting. This dynamic leadership role blends clinical expertise, operational oversight, and compassionate coordination to ensure our patients receive the care they need-when and where they need it most. You'll mentor a team of skilled nurses, therapists, and aides, while acting as the bridge between clinical staff, patients, families, and administration.

Key Responsibilities

  • Team Leadership : Hire, train, supervise, and support a multidisciplinary team. Conduct performance evaluations and foster a culture of excellence and accountability.
  • Patient-Centered Care : Oversee and guide individualized care plans in collaboration with physicians and other care partners to ensure timely, effective interventions.
  • Regulatory Compliance : Ensure adherence to all federal, state, and agency regulations, including Medicare / Medicaid guidelines and Joint Commission standards.
  • Quality & Performance Improvement : Conduct audits, monitor outcomes, and implement strategies to continuously elevate the quality of care.
  • Advocacy & Support : Serve as a trusted point of contact for patients and families, addressing concerns with empathy and efficiency.
  • Clinical Documentation : Review and approve documentation for accuracy, completeness, and compliance.
  • Budget & Resource Management : Assist in budget oversight and resource allocation while maintaining high standards of care.
  • Staff Development : Deliver ongoing education on clinical best practices, regulatory changes, and internal protocols.
  • Cross-Functional Collaboration : Partner with administrative and operational teams to drive care coordination and patient satisfaction.

Requirements

What You'll Bring

  • Education : Bachelor of Science in Nursing (BSN) required; Master's in Nursing or Healthcare Administration preferred.
  • Licensure : Current RN license in the state of practice.
  • Experience : 2-3 years in home health or a related clinical setting, with proven supervisory or management experience.
  • Skills :
  • Inspirational leadership and team development
  • Strong interpersonal and communication skills
  • Solid understanding of home health regulations and reimbursement (Medicare, Medicaid)
  • Proficiency with electronic health records (EHR) and clinical documentation
  • Critical thinking and sound decision-making under pressure
  • Certifications : BLS required; Home Health Care Certification preferred
