About the Role
As the Clinical Manager, you'll oversee the delivery of high-quality, patient-centered care in a home setting. This dynamic leadership role blends clinical expertise, operational oversight, and compassionate coordination to ensure our patients receive the care they need-when and where they need it most. You'll mentor a team of skilled nurses, therapists, and aides, while acting as the bridge between clinical staff, patients, families, and administration.
Key Responsibilities
- Team Leadership : Hire, train, supervise, and support a multidisciplinary team. Conduct performance evaluations and foster a culture of excellence and accountability.
- Patient-Centered Care : Oversee and guide individualized care plans in collaboration with physicians and other care partners to ensure timely, effective interventions.
- Regulatory Compliance : Ensure adherence to all federal, state, and agency regulations, including Medicare / Medicaid guidelines and Joint Commission standards.
- Quality & Performance Improvement : Conduct audits, monitor outcomes, and implement strategies to continuously elevate the quality of care.
- Advocacy & Support : Serve as a trusted point of contact for patients and families, addressing concerns with empathy and efficiency.
- Clinical Documentation : Review and approve documentation for accuracy, completeness, and compliance.
- Budget & Resource Management : Assist in budget oversight and resource allocation while maintaining high standards of care.
- Staff Development : Deliver ongoing education on clinical best practices, regulatory changes, and internal protocols.
- Cross-Functional Collaboration : Partner with administrative and operational teams to drive care coordination and patient satisfaction.
