Job description
1. • Manage a dedicated caseload capped at 25 active Confident Futures participants. • Deliver personalised, holistic and health-informed coaching support. • Complete comprehensive needs assessments and co-produce tailored support plans. • Support participants to build confidence, resilience, routine, self-management and work readiness. • Deliver interventions aligned to the Confident Futures Health Confidence model. • Use coaching methodologies including motivational interviewing, strengths-based approaches and trauma-informed practice. • Support participants to identify and overcome barriers relating to health, wellbeing, confidence, housing, finances, caring responsibilities and social isolation. • Facilitate access to employment, volunteering, training, skills and community opportunities. • Support participants to develop realistic progression goals linked to future employment aspirations. • Maintain the Health Confidence Passport and all participant records. • Conduct regular face-to-face appointments, ensuring meetings are held at least every four weeks where appropriate. • Deliver appointments from both office locations and community outreach venues. • Ensure all contractual milestones and customer service standards are achieved. • Maintain accurate case notes and management information systems. • Meet performance expectations and quality standards set by the Service Delivery Manager You may be required to perform one or more of the activities below as required 2. • Deliver individual and group-based interventions focused on: • Think Well • Move Well • Sleep Well • Connect Well • Live Well • Work Well • Support participants to improve confidence, participation and readiness for work-related activity. • Encourage sustainable behavioural change through structured goal setting and action planning. 3. • Deliver support from community venues and outreach locations in addition to Maximus offices. • Increase accessibility for participants who may struggle to engage with traditional office-based services. • Represent Maximus professionally within local communities and partnership networks. • Ensure all outreach activity complies with safeguarding and health and safety requirements. 4. • Support participants to explore realistic employment pathways. • Provide guidance around workplace adjustments and disability confident employment practices. • Prepare participants for future employment, volunteering, skills development and vocational opportunities. • Maintain links with employers and community organisations where appropriate. Desirable • Level 3 or above qualification in Advice & Guidance, Employability, Coaching, Health & Social Care, Community Development or related discipline. • Level 3 Award in Information, Advice and Guidance (IAG). • Mental Health First Aid qualification. • Coaching qualification. • Experience supporting customers with disabilities, long-term health conditions or mental health needs. • Knowledge of DWP programmes and welfare-to-work provision. • Understanding of the LCWRA customer group. • Experience delivering health-informed employment support programmes. • Experience of outreach or community-based service delivery.
