Job Description

Supports Primary Care physicians / Health Home teams in identifying and behaviorally intervening with Health Home enrollees who could benefit from behavioral intervention.

Minimum Requirements

Master’s Degree in Social Work, Counseling or Psychology.

Current State of Missouri License as Clinical Licensed Social Worker (LCSW) or Licensed Professional counselor (LPC).

3+ years experience working in a healthcare related setting.

Good written and verbal communication skills.
