Description
The Homeless Outreach Coordinator – Public Health is responsible for developing, coordinating, and implementing special projects for various departments within the county. The projects will include research, strategic planning, and communication in various forms. The special projects coordinator will work with various stakeholders to accomplish goals specific to assigned projects.
This position operates during our five-day work week (Monday-Friday). This position has the ability to operate in a hybrid function with remote work and in-office work as needed.
Target Hire Range : 27.00 – 33.00 USD Hourly
Compensation will be determined based on education, experience, and skills.

Essential Duties :
- Team Leadership : Will act as the Homeless Outreach subject matter expert and team lead during twice weekly outreach and will engage with and assist in-house participants three days a week with a variety harm reduction / syringe service assistance.
- Comprehensive Outreach Strategy : Develop and execute a comprehensive team-based outreach strategy for unhoused individuals, with a focus on people who use drugs (PWUD). Collaborate with local shelters, service providers, and community organizations to ensure a coordinated approach that identifies and engages individuals in need, focusing on vulnerable populations, including veterans, youth, and those with mental health challenges.
- Supportive Services : Expand linked access to a range of supportive services, including mental health counseling, substance use disorder treatment, job training, and housing assistance. Collaborate with healthcare providers and social workers to address the unique needs of homeless individuals.
- Community Engagement and Advocacy : Build and strengthen relationships with the local community and advocate for policies and initiatives that address homelessness. Work closely with local government, non-profits, and the private sector to raise awareness and promote systemic change.
- Data Collection and Reporting : Establish data collection mechanisms to track outreach efforts, participant outcomes, and resource utilization. Analyze data to assess program effectiveness and report regularly to stakeholders and funders.
