No one is ever turned away from our clinics based on insurance status or ability to pay. We also offer supportive services such as case management, health education and benefits enrollment.Job SummaryThe Digital Health Navigator serves as a Community Health Worker (CHW) with a specialized focus on bridging the digital divide in healthcare. This role is designed for a tech-savvy individual passionate about helping patients safely navigate digital health resources. The Digital Health Navigator will assist patients in creating and maintaining MyChart accounts, accessing reliable medical information online, and leveraging digital tools to manage their health from home. Working collaboratively with clinical teams and community partners, the Digital Health Navigator will provide education, technical support, and one-on-one guidance to empower patients in using technology for better health outcomes.ESSENTIAL FUNCTIONS AND DUTIESDigital Health Education & Support : Assist patients in setting up and managing MyChart accounts to schedule appointments, view medical records, and communicate with providers.Teach patients how to safely search for accurate medical information online, recognizing credible sources.Provide guidance on using telehealth platforms, mobile health apps, and wearable health technology.Train patients on basic digital literacy (e.g., using email, patient portals, video calls, and health apps).Patient Navigation & Engagement : Support patients in scheduling follow-up visits, receiving appointment reminders, and accessing self-management resources.Work with clinical teams to help patients understand their health conditions and develop strategies for self-care using digital tools.Provide culturally appropriate health education in individual or group settings.Reinforce chronic disease management and preventive screening guidelines using digital resources.Community & Outreach Activities : Staff resource tables at community events, health fairs, and outreach programs to promote digital health literacy.Provide referrals to social services, community resources, and technical assistance programs for digital access.Attend and participate in relevant community meetings and health initiatives.Data & Quality Improvement : Support grant-related and quality health improvement projects, such as digital health literacy initiatives.Collect and enter data accurately using various software programs and patient registries.Track patient engagement and evaluate the impact of digital health education efforts.EDUCATION AND EXPERIENCE : High school diploma or Associates Degree. Completion of the CHW training program offered by Milwaukee AHEC or through an equivalent state program. Experience in a community-based agency, community health organization or related field. Valid Wisconsin driver's license, use of an automobile and current auto insurance. Basic computer skills and experience in data entry using Excel, Access or equivalent computer background.It is mandatory that all employees, employed providers and volunteers are vaccinated.
