In this role, you will help shape programs that support individuals living with chronic conditions while driving measurable improvements in care quality and access. As our Population Health Manager, you will proactively work to improve the health status of populations living with chronic conditions by implementing and promoting wellness initiatives and evidence-based interventions. You will develop and maintain population health policies, collaborate with providers and community partners, and educate teams on best practices aligned with NCQA and Medicare standards. You will oversee vendor relationships, monitor compliance, and ensure programs meet regulatory requirements while supporting disease management initiatives and quality improvement efforts. You will analyze data quarterly to identify gaps, implement enhancements, and report outcomes. You will also oversee the Health Information Line, update educational materials, and participate in wellness assessments and community outreach activities to support healthier lifestyles.
