Job Summary
This position is a collaborative member of the Integrated Case Management team working with clients of CCEWs Permanent Supportive Housing (PSH) programs and will provide direct support to staff and clients of ICM programs through training, resource connections and wrap-around service supports for program clients, with the goal of helping stabilize individuals and families, with a particular focus on health-related outcomes.
Eastern Washington provides PSH services for seven buildings in the Spokane area five for singles, and two for families. This position will work with individuals and families throughout this PSH portfolio. The expected program outcomes include but are not limited to 1) reduced returns to the emergency department after program clients are placed in PSH units, 2) Improved self-perception of wellness for clients and 3) Improved Social Determinants of Health for program clients.

Education / Experience : to perform this job successfully, an individual must have a masters degree in Social Work (MSW) or are scheduled to graduate within two months. One year minimum work experience in a healthcare setting. Must demonstrateknowledge of healthcare treatment and intervention modalities, community resources, clinical assessment and evaluation, treatment planning and the social determinants of health.
Certificates / Licenses : To perform this job successfully, an individual must have a valid Drivers Licenseand successfully pass background check applicable to position.
