Location : Brecon Village Campus, Saline, MI Schedule : Full-Time, Monday - Friday, with On-Call Rotation Take your leadership and caregiving skills to the next level at EHM Senior Solutions EHM Senior Solutions is seeking a compassionate and organized Healthcare Coordinator to support and oversee the care team at Brecon Village , including our licensed AFC Memory Support Center. Key Responsibilities Team Leadership : Provide daily oversight of care staff to ensure exceptional quality of care for residents. Staffing and Scheduling : Ensure sufficient staffing levels, support scheduler, and participate in the on-call rotation. Resident Advocacy : Address resident and family concerns with empathy and professionalism. Team Support : Liaise / Collaborate with multidisciplinary healthcare team to support resident care Care Team Collaboration : Facilitate daily huddles with the care team to ensure seamless coordination and communication. Care Analysis : Conduct resident assessments / evaluations and participate in care conferences. Training and Development : Assist in creating and delivering orientation and training programs to support care staff. Quality Initiatives : Contribute to quality improvement projects and maintain compliance with regulatory standards. Care Planning : Collaborate with the care team to create, update, and monitor individualized care plans for residents. Qualifications Required : Active LPN license in the State of Michigan. Experience in assisted living or memory care settings. Strong understanding of Alzheimers, dementia, and progressive memory care needs.
