Key Position Details :

• Full-time position
• 8-hour, day shifts
• No regular weekend rotation
• Position supports clients in Dakota County
• Must be located within commuting distance to the Allina Commons and surrounding areas
• Case Manager will visit with clients throughout the week and will also attend onsite meetings at the Commons or other sites

Job Description :

Provides ongoing case management and service coordination for clients by identifying needs and partnering with each to develop individualized care plans. Collaborates with interdisciplinary team regarding ongoing evaluation of client needs and status and adjust service plan and community supports as needed.

Principle Responsibilities

Develops individual treatment plan and goals.

Identifies barriers and develops strategies to overcome barriers.

May participate in multidisciplinary care conferences.

Pay Range

Pay Range : $30.39 to $41.65 per hour

The pay described reflects the base hiring pay range. Your starting rate would depend on a variety of factors including, but not limited to, your experience, education, and the union agreement (if applicable). Shift, weekend and / or other differentials may be available to increase your pay rate for certain shifts or work.
