Job Summary

As a Registered Nurse in the Hospital at Home program, you will deliver personalized, high-quality acute care to patients in their own environments. You will assess clinical and environmental needs, foster a healing atmosphere, and provide education and emotional support to patients and their families.

Key Responsibilities

• Acute Care Delivery : Provide comprehensive, high-quality acute care services to patients enrolled in the Hospital at Home program.
• Assessment & Care Planning : Conduct thorough assessments and develop individualized care plans tailored to each patient's unique needs.
• Patient & Family Education : Act as a key educator, offering ongoing support and resources to patients and their families to facilitate effective home care management.
• Interdisciplinary Collaboration : Work closely with interdisciplinary teams to coordinate care and optimize patient outcomes.
• Supportive Environment : Spend additional time at bedside to assess needs and create a supportive, healing environment.

Qualifications

Current and valid Registered Nurse (RN) license.
