How You Will Make a Difference

As a Care Transition Navigator, you will :

• Contribute to a culture of accountability and teamwork
• Deliver exemplary care to patients along the care continuum
• Cultivate and nurture relationships with referral sources
• Champion communication among and between care teams
• Own individual sales goals

Compensation / Earning Potential : salary range starting at $85,000 with unlimited commission potential
