Make a significant impact every day as a CenterWell Home Health nurse. You will deliver personalized, one-on-one care that empowers patients to regain independence in their own homes. Collaborating closely with a dedicated team of physicians and clinicians, you'll design and manage care plans tailored to support recovery and help patients return to the lives they cherish.
As a Home Health Registered Nurse, your responsibilities will include :
- Conducting admissions, case management, and follow-up skilled nursing visits for home health patients.
- Providing continuous care and case management for each patient, ensuring necessary follow-up as directed by the Clinical Manager.
- Collaborating with physicians to develop the initial treatment plan based on their orders and initial patient assessments. Deliver hands-on care and evaluate the care plan while educating patients in alignment with physician directives, all under the supervision of the Clinical Manager. Adjust plans based on ongoing assessments and regulatory requirements in conjunction with the physician.
- Coordinating comprehensive care that incorporates various healthcare professionals such as Physical Therapists, Occupational Therapists, and Home Health Aides.
- Reporting continuous updates on patient care, condition, and progress to the physician and Clinical Manager.
- Implementing the patient care plan alongside the patient and their family to achieve optimal resolution of needs and challenges.
- Overseeing the work of Home Health Aides, Certified Home Health Aides, and Personal Care Workers, providing detailed personal care instructions that reflect the current care plan. Monitoring the accurate completion of documentation by home health aides as part of supervisory duties.
- Discharging patients after consulting with the physician and Clinical Manager, ensuring comprehensive clinical documentation is prepared and completed.
- Compiling necessary medical documentation for all patients, including case conferences, patient contacts, medication changes, re-certifications, progress updates, and care plan modifications. Generating visit reports that summarize patient records and collaborating with other healthcare professionals to facilitate optimal patient care.

Requirements :
Compensation : $45.00 - $63.00 per visit / unit; $70,500 - $96,900 annual base pay.
Hours : Scheduled weekly hours : 1.
Salary Range : $77,200 - $106,200 per year reflects a good faith estimate of starting base pay for full-time (40 hours per week) employment. Actual compensation may vary based on geographic location and individual skills, knowledge, experience, education, certifications, etc.
