AuthoraCare Collective is currently seeking an RN Home Health for ourIntegrated Health Services (Skilled home-based-care) department. This position is Full-time : Monday - Friday 8 : 00am - 5 : 00pm.

The RN Home Health will Manage the overall Plan of Care for a case load of Integrated health patients in collaboration with other members of the interdisciplinary team (IDT). Assess and identify patient / caregiver problems, establish nursing goals, and interventions. Our RN Home Health will work with physician to establish medical Plan of Care for the patient. They will establish and maintain therapeutic relationship with patient, including providing disease education, medication management, and health recovery.

The RN Home Health will work closely with other members of the interdisciplinary team and communicate effectively both verbally and in writing with team members, patients, caregivers, physicians, pharmacists, and other health care professionals. Our RN Home Health will document the goals and outcomes of care.Theyassume on call responsibilities as scheduled.
