Join a dynamic healthcare team delivering high-quality primary and diagnostic care in nursing home and long-term care settings. This role focuses on comprehensive medical management, continuity of care, and collaboration with interdisciplinary teams to optimize patient outcomes.

💰 Salary Range: $200,000 – $230,000

🌟 Key Responsibilities

  • Provide primary medical care to nursing home patients in accordance with standards
  • Assess, diagnose, and manage acute and chronic medical conditions
  • Order and interpret laboratory and imaging studies; refer to specialists as needed
  • Prescribe medications and treatment plans, including lifestyle and dietary guidance
  • Educate patients on treatment options and medication side effects
  • Maintain accurate, timely, and compliant documentation
  • Collaborate with multidisciplinary teams to ensure optimal patient outcomes
  • Adhere to quality improvement, safety, and infection control standards

Requirements

  • MD or DO from an accredited U.S. medical school
  • Board Certified or Board Eligible in Family Medicine, Internal Medicine, Geriatrics, or Palliative Care/Hospice
  • Completion of an accredited U.S. residency (or equivalent pathway)
  • Active New York State medical license (or eligibility during credentialing)
  • DEA registration (or ability to obtain during credentialing)
  • Experience providing primary and diagnostic care in a clinical setting
