As a field-based CNA, you'll serve patients in their home or care setting by providing essential personal care services under the supervision of a registered nurse. You'll work closely with families, support the care team, and help bring comfort and peaceone visit at a time.

Your Role in the Mission

As a Certified Nursing Assistant (CNA) at Lifted Hospice, you play a crucial role in bringing dignity, comfort, and consistency to our patients and their families. You are often the most present member of the care teamthe one who helps a patient feel seen, safe, and supported every day.

Key Responsibilities Include:

  • Following the Plan of Care Providing direct personal care under the supervision of the RN, in alignment with the CNA Plan of Care and patient preferences.
  • Assisting with mobility and safety Helping patients transfer, move, and ambulate safelywhile maintaining a calm, clean, and secure environment.
  • Observing and reporting Noticing and promptly reporting any changes in the patient's condition or behavior to the RN, supporting proactive care.
  • Practicing infection control Following all safety and hygiene protocols to protect the health of patients, families, and teammates.
  • Respecting privacy and dignity Upholding patient confidentiality, respecting personal property, and communicating with kindness and professionalism.

Physical Requirements & Work Environment

  • Remain active throughout the day Ability to stand, walk, bend, and move frequently while providing patient care.
  • Assist with patient mobility Ability to safely lift up to 50 pounds and assist with transferring, repositioning, and moving patients.
  • Work in a fast-paced environment Ability to prioritize tasks, make sound clinical decisions, and respond calmly to changing patient and family needs.
  • Use clinical equipment Demonstrate manual dexterity, hand-eye coordination, and the ability to safely operate durable medical equipment.

Skills and Qualifications

  • Completion of a state-approved Certified Nursing Assistant (CNA) training program.
  • Active CNA certification in the State of Texas.
  • High school diploma or equivalent required.
  • At least one year of experience in hospice, home health, or a similar care setting is preferred.
  • Compassionate presence and strong communication skills.
  • Basic understanding of hospice care principles and a heart to serve.
  • Ability to read, write, and communicate clearly in English.
  • Comfortable using a smartphone for communication and documentation.
  • Valid Texas driver's license, reliable transportation, and current auto liability insurance

Where You'll Work

In this role, you'll serve patients wherever they call homewhether in a private residence, assisted living, nursing facility, or memory care setting.

You'll provide personal care in varied environments, both indoors and outdoors, and may be exposed to infectious diseases and bodily fluids as part of your essential work. You'll also regularly connect with your local care team to stay aligned on patient needs and ensure care is consistent, respectful, and personalized.
