Located in beautiful Louisville, CO, our 104-bed facility provides a peaceful setting for those seeking treatment for mental illness including chemical dependency. Centennial Peaks offers unique and individualized programming for adolescents and adults that sets us apart from many other treatment facilities, and our dynamic leadership team includes a seasoned nursing department, ECT Therapy, Intensive Outpatient and Partial Hospitalization Programs.

The Social Worker provides clinical social work services on units as assigned in a manner consistent with the philosophy and objectives of Centennial Peaks Hospital. The Social Worker is responsible for psychosocial assessments, individual, group and family therapy, participation in treatment team planning, processing commitments, and discharge planning/case management service, crisis intervention and liaison services with referral sources and other departments within the hospital.
This position is eligible for our generous shift differentials: Weekend Day (Sat-Sun) $2/hr, Evening (Mon-Thurs) $3.25/hr, Weekend Evening (Fri-Sun) $5.00/hr, Night (Mon-Thurs) $5.00/hr, Weekend Night (Fri-Sun) $7.00/hr. *must work three and a half hours (3.5 hours) into a shift to qualify for shift differential.
PRN Shift Differential: $1.50/hr

Qualifications
Requirements:
- The ability to multi-task and thrive in a fast-paced environment is critical.
- Masters degree in Social Work, Clinical/Counseling Psychology or social services related field required, licensure preferred.
- 1 to 2 years mental health experience preferred. Prior experience working with a psychiatric mental health inpatient setting preferred.
- License eligible degree;LCSW, LMFT, LPC preferred.
- Prefer knowledge of case management, discharge planning, group leadership skills, age specific growth and development, crisis and behavior management, family therapy theory and practice.
- Social Worker must demonstrate flexibility for reassignment to units providing care to varied populations including, adults, adolescents, and dually diagnosed patients in an inpatient and partial hospital setting.
This is a non-exempt position; hours vary based on staffing and patient needs and include day, evening, and weekends.
