OVERVIEW
Kitsap County Human Services Division of Aging and Long-Term Care has an exciting opportunity for a Care Coordinator to join our team to assist older adults with hospital discharge planning as part of the Care Transitions program.
In this role, you will partner with local hospitals to support non-Medicaid acute care discharges and in-home long-term care referrals to assist with care transitions from hospital to home.
Our new team member will have knowledge of the local aging service delivery systems and other health and human services delivery systems. You should share our philosophy of coordinating care in a client-centered manner, and have the ability to manage complex interventions across diverse professional disciplines and service systems. You must have professional case management ethics and the ability to establish and maintain a professional demeanor and effective working relationships.

QUALIFICATIONS FOR PERFORMANCE OF THE ESSENTIAL FUNCTIONS
Required Education and Experience
- Bachelor's degree in behavior or health sciences AND two years of paid, on-the-job social service experience OR
A general Bachelor's degree from a college or university accredited by an agency recognized by the U.S. Department of Education or equivalent AND four years of paid, on-the-job social service experience.

Category 2 : Driver operates a personal vehicle as an essential function for official County business.

ILLUSTRATIVE EXAMPLE OF DUTIES
Conduct comprehensive assessment (including the physical, psychological, and social needs of the individual)
Maintain client records
Develop and implement the goals identified in the service plan to mobilize the formal and informal resources and services identified in the assessment to meet the needs of the individual, including coordination of the resources and services
Coordination and monitoring of formal and informal service delivery, including coordination and monitoring to ensure that services specified in the plan are being provided
Access & actively utilize a resource database including remaining current on local service providers, applying basic research tools to assist clients with accessing services outside of Kitsap County, and conducting research regarding new services
Assist with representing the Aging & Long Term Care staff with community events, outreach, and public information activities

This recruitment is being used to fill an existing open position, and may be used to fill future openings across all Aging divisions for up to six (6) months.
Work hours for this position are typically Monday - Friday, 8 a.m. - 4 : 30 p.m.

Holidays
11 paid holidays and 2 additional floating holiday per year. One additional floating holiday after 10 years of service.
Vacation Leave
New employees are credited with 48 hours of annual leave upon hire and will be begin accruing at the rate of 8 hours per month starting on their 7th month of employment. Employees may carry over 360 hours from one calendar year to the next. Annual leave accrual will increase upon tenure, as follows :
Upon completion of 2 years : 15 days per year (10.00 hours per month)
Upon completion of 5 years : 20 days per year (13.33 hours per month)
Upon completion of 10 years : 25 days per year (16.67 hours per month)
Sick Leave
All new hires will be credited with 48 hours of sick leave upon hire and will begin accruing at the rate of 8 hours per month starting on their 7th month of employment. Employees may carry over 1200 hours from one calendar year to the next.
