POSITION SUMMARY
The Pediatric Social Worker provides specialized bio-psycho-socio-cultural care to pediatric patients and their families, ensuring comprehensive support in end-of-life care, chronic illness management, and emotional well-being. This role is crucial in delivering age-appropriate counseling, advocacy, and crisis intervention, guiding families through complex medical and social challenges.
Primary Responsibilities
Comprehensive Pediatric Assessments
Conduct in-depth assessments tailored to pediatric patients and their families, addressing emotional, developmental, and social needs.
Develop and implement individualized therapeutic treatment plans, ensuring age-appropriate intervention strategies.
Therapeutic Counseling & Crisis Intervention
Provide age-specific counseling and therapy for pediatric patients and their families dealing with grief, illness adjustment, family dynamics, trauma, and bereavement.
Offer crisis intervention, helping families navigate medical decision-making, symptom management, and psychosocial stressors.
Family & Caregiver Support
Educate families on medical processes, Advanced Health Care Directives, and coping strategies for managing pediatric health conditions.
Facilitate family counseling to address emotional and behavioral challenges during critical life transitions.
Case Management & Community Advocacy
Coordinate referrals to pediatric-specific community resources, ensuring access to educational, social, and emotional support services.
Advocate for children and families, promoting comprehensive care solutions within medical and social frameworks.
Interdisciplinary Collaboration & Care Planning
Work closely with medical teams, pediatric specialists, therapists, and educators to develop holistic, child-centered care plans.
Participate actively in Interdisciplinary Group (IDG) meetings, ensuring consistent communication and strategy alignment across care teams.
Training & Professional Development
Assist in the training and orientation of new staff, ensuring best practices for pediatric psychosocial care.
Engage in regular clinical supervision and continuing education focused on pediatric social work.
Qualifications & Skills
Master’s degree in Social Work (MSW) from an accredited program.
Licensed Social Worker (LSW) in the State of Hawaii.
Minimum 1-3 years of pediatric social work experience, preferably in hospice, hospital, or child welfare settings.
Strong knowledge of child psychology, trauma-informed care, and bereavement counseling.
Expertise in family systems, pediatric grief support, and medical social work principles.
Ability to navigate complex family dynamics and provide emotionally responsive care.
Preferred Qualifications
Certification in Pediatric Social Work or Hospice & Palliative Care Social Work (APHSW-C).
Familiarity with nonprofit healthcare operations, grant funding, and advocacy programs.
Working Conditions
Generally, indoors in patient homes or nursing and acute care facilities; may include outdoors in all types of weather.
In-person patient visits, family meetings, and team collaborations.
Requires travel to pediatric care sites, schools, and family homes across Oahu.
Must adhere to strict safety, infection control, and HIPAA guidelines.
Hours :
Generally, Monday through Friday, 8 : 00 am through 4 : 30 pm.
Requires intermittent On-Call supervisory coverage.
