The goal of this role is ensure DaVita is the provider of choice to hospitals. This teammate will engage renal discharge planners and case managers in an effort to build and strengthen relationships. Their focus is to ensure a smooth and seamless experience in support of newly diagnosed and hospitalized dialysis patients to ensure a successful transition to the outpatient setting.Essential DutiesConsistent outreach to discharge planners to provide education and build awareness of offeringsIdentify opportunities to grow as provider of choice through relationship building and addressing feedbackPartner with internal field teammates to optimize the admission process and implement best demonstrated practicesAssist with necessary communication between hospital, admission and clinic teammates to work through patient placements with extenuating circumstancesSupport a smooth and seamless discharge experience for patients and hospital staff to DaVita centersDevelop comprehensive knowledge of local marketsSeeking candidates who demonstrate : Commitment to DaVita's values of Service Excellence, Integrity, Team, Continuous Improvement, Accountability, Fulfillment and Fun with ability to demonstrate these core values daily in performance and interactionsAbility to build and foster strong relationships with people across many levels and diverse backgroundsNatural curiosity with a growth mindsetSelf-starter with a strong bias for actionHigh accountability and follow through, with a passion for problem solvingStrategic mindset, with the ability to systematically connect dotsMinimum qualificationsBachelor's Degree preferredAbility to travel to various locations to meet with partners (internal and external) on an as-needed basis.
