Your Role as a PT : Care Planning : Screen, evaluate, and develop comprehensive treatment plans for residents to optimize their functionality and independence. Team Leadership : Supervise and collaborate with Physical Therapy Assistants (PTAs) to ensure efficient and effective care. Collaboration : Work closely with the interdisciplinary team (nursing, social services, etc.) and attend care conferences to coordinate holistic resident care. Documentation : Maintain timely and accurate electronic health records (EHR) and billing in accordance with state and federal regulations. Connection : Foster a warm, enthusiastic, and compassionate atmosphere for our residents and their families. Qualifications : Licensure : Graduate of an accredited Physical Therapy program with a valid State Physical Therapist license (or eligibility to obtain one). Experience : Previous therapy experience preferred; experience in long-term care (LTC) or skilled nursing is a plus. Skills : Proficiency with computers and Electronic Health Records (EHR). Attributes : Strong organizational, critical thinking, and time management skills. Availability : Flexible availability to work weekends, holidays, or travel to sister facilities if needed. Physical Requirements : Ability to lift / carry 50 lbs and perform the physical tasks associated with patient transfer and therapy.
