Summary
The Community Living Center (CLC) which includes Hospice / Palliative Care Rehab Short Stay Skilled Care and Long-Term Care is seeking a Licensed Practical / Vocational Nurse (LPN / LVN). The LPN / LVN will perform assigned duties to promote quality health care and positive customer service while maintaining an environment dedicated to serving veterans. The role requires delivering outstanding customer service and ensuring the provision of high-quality health care to veterans.
Duties
VA Careers - Licensed Practical Nurse : LPN performs reports and documents assigned nursing care in the Community Living Center team including but not limited to :
- Work in Hospice / Palliative Care Rehab Short Stay Skilled Care and Long-Term Care units
- Engage with the HATCH model focusing on the needs interests and choices of residents patients and clients
- Specialize in treating geriatric veterans with long-term skilled nursing and / or specialty needs
- Manage residents with transitory mental status changes or secondary psychiatric diagnoses
- Operate under supervision of a nurse manager charge registered nurse or other supervisor
- Receive administrative supervision general instructions and work assignments from the nurse manager or charge registered nurse
- Follow assigned time schedules leave approvals and performance evaluations by the nurse manager or charge registered nurse
- Obtain clinical supervision from a registered nurse physician or other provider
- Function under the direct supervision of the registered nurse responsible for coordinating resident care
- Administer medications according to policies and procedures
- Document observations treatments procedures and conditions electronically via CPRS and BCMA
- Verbally report observations and conditions to the appropriate person typically the registered nurse
- Carry out recurring duties independently and seek guidance when necessary
- Provide age-appropriate nursing care to residents
Work Schedule : 7 : 00 am - 7 : 30 pm rotating including weekends and holidays

Pay : Competitive salary and regular salary increases When setting pay a higher step rate of the appropriate grade may be determined after consideration of higher or unique qualifications or special needs of the VA (Above Minimum Rate of the Grade).
Paid Time Off : 37-50 days of annual paid time offer per year (13-26 days of annual leave 13 days of sick leave 11 paid Federal holidays per year)Selected applicants may qualify for credit toward annual leave accrual based on prior work experience or military service experience.
Parental Leave : After 12 months of employment up to 12 weeks of paid parental leave in connection with the birth adoption or foster care placement of a child.
Child Care Subsidy : After 60 days of employment full time employees with a total family income below $144000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66 . Retirement : Traditional federal pension (5 years vesting) and federal 401K with up to 5% in contributions by VA
Insurance : Federal health / vision / dental / term life / long-term care (many federal insurance programs can be carried into retirement)

Basic Requirements :
May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).
Grade Determinations : GS-3. None beyond the basic requirements. LPNs at this grade level are expected to perform all duties within the scope of their license (e.g. medication administration); however completed work may require routine or detailed higher level review depending upon the complexity of the duties involved.
GS-4. Employees may be appointed at or advanced to GS-4 if they meet any of the criteria below :

Preferred Experience : A minimum of 2-3 years of LTC experience is preferred.

Physical Requirements : Extensive periods of sitting standing and walking. Working with patients requires regular and recurring lifting pushing stooping squatting kneeling crouching and similar activities. See VA Directive and Handbook 5019.
