We are currently seeking a dedicated Licensed Practical Nurse (LPN) to join our team and provide high-quality care that meets the physical, emotional, and medical needs of our residents.

Key Responsibilities :

• Administer medications and treatments as prescribed by physicians
• Monitor and record vital signs, fluid intake / output, and residents’ overall health status
• Assist in the development, implementation, and evaluation of individualized care plans
• Provide personal care (bathing, dressing, grooming) and support activities of daily living
• Observe residents for any changes in condition and promptly report concerns to the RN or physician
• Collaborate with RNs, CNAs, therapists, and other team members to coordinate resident care
• Maintain accurate, timely clinical documentation in accordance with state and federal regulations
• Educate residents and families on health care needs, medication management, and facility policies
• Supervise and mentor Certified Nursing Assistants (CNAs) as assigned
• Ensure compliance with Hawaii state regulations (OHCA) and facility policies
