LPN Homecare Nurse – Rockford, IL

Maxim Healthcare Services is seeking compassionate and dedicated Licensed Practical Nurses (LPNs) to provide one-on-one care for adult and pediatric patients in the comfort of their homes throughout the Greater Rockford, Illinois area.

Patients may require skilled nursing support, including G-tube, tracheostomy, and ventilator care. No homecare experience? We provide paid training and ongoing clinical support to help you succeed.

Full-time and part-time opportunities available.

Position Responsibilities

• Utilize the nursing process to assess, plan, implement, and evaluate patient care
• Develop and update individualized plans of care based on patient needs
• Deliver therapeutic, preventive, and rehabilitative nursing interventions
• Monitor patient conditions and communicate changes to physicians and supervisors
• Administer medications and prescribed treatments
• Document care and evaluate treatment effectiveness
• Educate patients and families on healthcare needs and treatment plans
• Collaborate with the care team to support care coordination, goal setting, and discharge planning

Qualifications

Required

• Current Illinois Licensed Practical Nurse (LPN) license
• Current BLS/CPR certification
• Current Health Certificate (as required by state regulations)
• Current PPD or Chest X-Ray

Preferred

• One year of LPN experience
