AvaMed Inc is a Healthcare Staffing Agency that staffs all of California State Correctional Facilities. At the moment we are in URGENT need of a Licensed Professional Clinical Counselor to take on a Full-Time assignment at the correctional facility located in Lancaster, San Diego, Wasco, Stockton, Delano, Chowchilla, Coalinga, Corcoran, Ione, Susanville, Represa, and Crescent City. Travelers and locals are welcome. Provider must be a LICENSED in California by Board of Behavioral Science Position Details : Full - Time Flexible Shifts and Hours $50-$53 / hr (W2 / traveler) Job Description is as follows but is not limited to : a. Periodically serve on-call during scheduled workdays for patient emergencies, provide consultation and crisis intervention services on an as-needed basis. b. Conduct initial mental health (MH) evaluations including criminal, psychological, and substance abuse case history to assess patient current needs and make treatment recommendations in the initial evaluation. c. Conduct clinically sound suicide risk evaluations, including clinical review, applying suicide risk assessment protocols, pertinent data / chart reviews, proper documentation, and consultations as needed. d. Meet with patients for crisis intervention, group psychotherapy, in-person interviews, etc. The clinician may be required to provide clinical, in-person interviews at cell fronts in inmate housing units. In some institutions / facilities, due to physical plant layout and multi-tiered housing units, the ability to climb stairs and walk long distances may be required. e. Identify the relevant signs and symptoms of psychiatric disorders in order to comprehensively assess, diagnose and manage the presenting symptoms, consults with medical personnel regarding the findings of medical examinations and evidence of organic disturbances related to behavior disorders, and with nursing, social work, education, custody and other staff regarding precipitating events, complications, and potential for adjustment. f. Write clinical reports regarding diagnoses and prognoses and develop treatment plans that reflect the standard of practice. g. Consult on complex cases with supervisor, treatment team members, CDCR / CCHCS institutional health care personnel, and other divisions within the department. h. Document all care provided to the patient, including in-person contacts and Interdisciplinary Treatment Team (IDTT) meetings. i. Monitor patient progress using CDCR / CCHCS MH approved documentation formatting for progress notes. j. Effectively apply community standards of practice for non-medical treatment modalities and ethical standards. k. Perform mental status exam (MSE) in-person clinical interviews. l. Examine patient information from electronic medical record, inpatient record, custody file,institutional chronos, probation reports, court records, and other available documents to assist in the formulation of patient diagnosis and aid in the development of patient treatment plans. m. Conduct health education and develop additional resources for patients. n. Assist patients in applying for entitlement benefits, community programs, and services. O. Facilitate patient transition between various settings within the institution / facility. p. Perform pre-release functions for releasing patients, to include patient education, while facilitating transition to county MH, county medical services and community MH treatment programs including county addiction services; coordinate discharge planning activities and act as resource on accessing appropriate community support and services to be utilized upon release. q. Perform medical social work functions as needed. r. Participate in and / or lead IDTT and other multidisciplinary meetings, develop, monitor, and modify individualized patient treatment plans in collaboration with the interdisciplinary treatment team,identify and recommend appropriate services and level of care based on assessment and, where applicable, civil or penal code commitment. s. Develop and distribute draft and final discharge plans. t. Provide information (e.g., appointment documentation, no-shows, etc.) to support staff for data entry. u. Provide written reports and consult with the Board of Prison Hearings and other CDCR / CCHCS staff. v. Provide relatives / caregivers with information concerning patients in person, by phone, or by correspondence as approved by the supervisor and only after receiving the patient's signed release of information for that specific individual. w. Develop a therapeutic relationship with the patients, families, and caretakers as assigned by the supervisor in order to enhance effective treatment delivery. x. Represent the department at formal and informal settings regarding medical, MH, andaddiction matters related to clinical social work. y. Protect and maintain safety and security of property, including documentation of unusual occurrences and inventory control of psychological testing material, reference material, computers,and other equipment. z. Maintain order and supervise the conduct of persons committed to the institution / facility in order to prevent escapes or injury by these persons to themselves or others. aa. Give input and assist in the implementation of new programs for the treatment, training, or rehabilitation of patients. bb. Participate in quality / performance improvement activities, including Quality Improvement Teams (QITs) and Root Cause Analysis (RCAs), as appropriate. cc. Participate in Unit Classification Committees (UCC) and Institutional Classification Committees (ICC) when required.
