Housing Works fights for funding and legislation to ensure that all people living with HIV / AIDS have access to quality housing, healthcare, HIV prevention, and treatment, among other lifesaving services.

Compensation Range : $65,000 - $75,000 yearly commensurate with experience .

Overview :

CORE Licensed Mental Health Counselor Using a one to one and group person-centered counseling model, Community Oriented Recovery and Empowerment (CORE) program seeks to improve the functioning of individuals living with mental health diagnoses, histories of chemical and alcohol dependence, or physical disabilities, helping them achieve personal goals in the areas of recovery, employment or independent functioning.

Under the supervision of the Program Director, the Licensed Mental Health Counselor will provide CORE services to HARP eligible clients primarily for Community Psychiatric Support and Treatment (CPST) Services within the CORE Program and team. Community Psychiatric Support and Treatment (CPST) is individual and family interventions that include goal-directed supports and solution-focused interventions with the intent to achieve person-centered goals and objectives. CPST utilizes therapeutic interventions such as clinical counseling and therapy, which assist the individual in achieving stability and functional improvement. CPST addresses behavioral health barriers that impact daily living, finances, housing, education, employment, personal recovery and / or resilience, family and interpersonal relationships and community participation. CPST is designed to provide mobile treatment services to individuals who have difficulty engaging in site-based programs, or who have not been previously engaged in services, including those who had only partially benefited from traditional treatment. CPST services must be delivered offsite unless there is a clinical rationale for delivering services onsite on a limited basis.

The CORE Licensed Mental Health Counselor may also provide Psychosocial Rehabilitation (PSR) in addition to CPST services, as needed. Psychosocial Rehabilitation (PSR) is designed to assist an individual in improving their functional abilities to the greatest degree possible in settings where they live, work, learn, and socialize. Rehabilitation counseling, skill building, and psychoeducational interventions provided through PSR are used to support attainment of person-centered recovery goals and valued life roles.

CORE services are intended to contribute to the reduction of unnecessary hospitalization / institutional level of care and improved societal integration of those served. Service delivery is designed to be flexible and individualized and is focused on the promotion of skill development and identification of community supports in order to overcome deficits related to function and motivation.

Responsibilities :

  • Community Psychiatric Support and Treatment (CPST) Services :

Provide time-limited, goal directed interventions included in the ISP are time-limited, goal-directed support and solution-focused, with the intent to achieve stability and functional improvement.

  • Conducts Person-Centered Assessment, Service Planning, Documentation, and Collaboration
  • Provides Individual and Family Psychotherapy and Counseling and Integrated Co-Occurring Disorder Treatment
  • Actively engages around Crisis Prevention
  • Offers customized interventions for Intensive Rehabilitation counseling designed to intensely assist an individual in attaining and sustaining meaningful life roles.
  • Psychosocial Rehabilitation (PSR) Services :
  • Engages in person-centered planning process Works to support the individuals' skill building and psychoeducation to develop or improve personal coping strategies, prevent crises, and promote recovery

  • Provides Rehabilitation Counseling to improve functioning in living, working, learning, and social environments.
  • Maintain a caseload of clients providing 25 hours of clinical care weekly on average
  • Assist clients in articulating personal goals using client-centered approach
  • Demonstrated ability to work independently and as part of a team, including working collaboratively with professionals across disciplines, (e.g., domestic violence advocates, housing specialists, attorneys, Rehabilitation counselors, government officials, workforce development professionals and other community-based agency staff.)
  • The counselor will conduct intake assessment, counseling, advocacy, and referral services to HARP members
  • Opportunity to receive training in evidence-based modalities
  • Hybrid work requirement with both remote and in-person formats with a majority of work expected to be done in community with the clients
  • Provide services as outlined in each participant's Plan of Care
  • Assist clients with referral to other services and agencies determined to be necessary to the recovery process (i.e. shelters, detox units, legal counsel, etc.)
  • Collaborating with clients' care teams and providers to determine the steps needed in order to achieve individuals' goals and self-directed recovery.
  • Teach clients how to identify and combat negative self-talk and how to identify and overcome fears.
  • Support clients' vocational / educational choices and assist them in choosing a job that matches their strengths, overcoming job-related anxiety by reviewing job applications, and providing interview tips.
  • Maintain case records including progress notes and Individualized service plan in compliance with state regulatory requirements
  • Hold case conferences as needed
  • Secondary : Other duties as assigned
  • Minimum Requirements

    Educational Requirements :

  • Masters Degree in the Healing Arts with active license in one of the following to practice is required :
  • Licensed master social worker (LMSW)

  • Licensed Marriage and family therapist (LMFT)
  • Licensed Mental health counselor (LMHC)
  • Licensed Creative arts therapist (LCAT)
  • Recent graduates working towards licensure (with limited permit approved by NY State) may be considered
  • Experiences with serving people with mental health diagnoses or substance use preferred
  • The following additional competencies are viewed as important to success in this position :

  • Experience or sensitivity to workingwithindividuals or families who have issues of mental illness, substance use, justice involved, homelessnessand / or HIV / AIDS
  • Ability to use data to inform clinical and administrative decision making
  • Ability to apply population health and Continuous Quality Improvement principles to work
  • Must be able to remain in stationary position 50% of time.
  • Frequently move about inside the office to access file cabinets, paperwork, office machinery, retrieve clients from waiting area etc.
  • Frequently stoops, bends, or reach to access files or retrieve other documents.
  • Constantly operates a computer and other office productivity machinery such as calculator, copy machine and computer printer
  • Frequently communicates (exchanges information, converse with, express oneself to) with subordinates, vendors, supervisors, community at large, clients regarding treatment, management, incidents, reporting, advertise services.
  • Must be able to determine accuracy of reports (figures, balances, documentation such as case notes) perceive risk, discern agitation of clients and deescalate any situation, judge, observe and assess situations germane to residence / case management operations.
  • Excellent verbal and written communications skills; articulation and instruction of policy procedure and practice.
  • Frequently exchange information in communication with individuals who are agitated, confrontational or difficult clients and staff from a variety of socio-economic, cultural and religious background.
  • Occasionally move, position or put files weighing up to 10lbs.
  • Occasionally required to traverse public transportation in order to attend meetings, give presentations or meet with clients in community.
  • Noise level is moderate
  • This is Community based role with regulatory requirements that most work occurs in the field. The essential functions of this position may not be performed within the office.
  • Must operate cell phone or other technology to maintain consistent engagement and response to supervisor, co-workers, clients, community partners and other external care providers.
  • Occasional to frequent exposure to clients with infectious diseases.
