Job Description :
- Provides preventative and health care to an assigned group of patients or clients in a healthcare facility.
- Administers medications safely under the direction of a physician or other medical personnel.
- Observes, records, and reports patients' condition and reaction to drugs and treatments to physicians.
- Provides care according to an established nursing plan.
- Collects comprehensive data pertinent to the patient's health or situation.
- Ensures patient chart entries are accurate and timely, forwarding charts as appropriate.
- Communicates systematic reports to other care providers.
- Assists in the coordination of care delivery; utilizes resources to promote the continuum of care.
- Employs strategies to promote health in a safe environment.
- Documents progress toward the attainment of outcomes.
- Uses quality and evidence-based practices to deliver and improve care.
- Works closely with more experienced clinicians for guidance and supervision.
- Triages telephone calls for referral to other clinical team members.
- Performs other duties as assigned.
