Job Title: LPN

Location: Tulsa, OK

Job Summary:

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team at Tulsa Center for Rehabilitation & Healthcare, a skilled nursing and rehabilitation community serving Tulsa and the surrounding area. As an LPN at our facility, you will provide direct nursing care to our residents, supervising the day-to-day nursing activities performed by certified nursing assistants, and ensuring that our residents receive high-quality care that meets their individual needs.

Responsibilities:

  • Provides direct nursing care to residents, including administering medications, performing wound treatments, and providing assistance with activities of daily living (ADLs)

  • Supervises the day-to-day nursing activities performed by certified nursing assistants (CNAs) and medication technicians/aides

  • Completes accurate and timely documentation of resident care and services

  • Participates in the admission, discharge, and transfer process of residents

  • Observes and reports changes in residents' status to the physician and resident's family or representative

  • Collaborates with other members of the interdisciplinary team to ensure comprehensive care that meets the needs of our residents

  • Participates in quality improvement initiatives to enhance the quality of care and services provided

  • Maintains a safe and clean environment for residents, staff, and visitors

Required Qualifications:

  • A Nursing Degree from an accredited college or university or a graduate of an approved LPN/LVN program

  • Prior experience in long-term care preferred but not required

  • Current unrestricted license as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) in practicing state

  • Current CPR certification

Major Duties and Responsibilities:

  • Directs the daily activities of CNAs in accordance with current federal, state, and local regulations and guidelines and established facility policies and procedures

  • Ensures that policies and procedures are complied with by nursing personnel assigned

  • Participates in the survey process when inspection surveys arise

  • Maintains confidentiality of protected health information, including verbal, written, and electronic communications

  • Reports noncompliance with policies, procedures, regulations, or breaches in confidentiality to appropriate personnel

Additional Tasks:

  • Treats all residents with dignity and respect

  • Promotes and protects all residents' rights

  • Establishes a culture of compliance by adhering to all facility policies and procedures

  • Complies with standards of business conduct and state/federal regulations and guidelines

Personal Skills and Traits Desired:

  • Ability to read, write, speak, and understand the English language

  • Ability to make independent decisions when circumstances warrant such action

  • Ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public

  • Must possess leadership and supervisory ability and willingness to work harmoniously with and supervise other personnel

  • Must be able to follow oral and written instructions

  • Ability to communicate effectively with medical and nursing staff, and other departments

Compliance as a Condition of Employment and Performance Appraisal:

  • Agreement to abide by all standards, policies, and procedures of the facility, including the facility's compliance and ethics program, is a condition of employment

  • Compliance will be a factor in evaluating job performance
