NTSOC is looking for a compassionate LPN to provide high quality nursing care for our pediatric and adult clients to improve their quality of life at home.
Job Type : Full time and Part time
Compensation : $24.00-$33.00 per hour dependent upon experience.

Essential Functions :
- Provide one on one care to a single client in their home setting and while in the community.
- Implements the client's physician ordered plan of care and nursing interventions while adhering to agency policies.
- Perform assessments to monitor the health status of the client.
- Notify the physician and Field Nurse Case Manager of any changes in the client's condition.
- Coordinate care with the Field Nurse Case Manager and other members of the client's multidisciplinary care team.
- Administer medications and treatments as ordered.
- Document performed interventions and treatments.
- Assists clients with activities of daily living such as bathing, toileting, dressing, and grooming.
- Collaborate with the care team on implementation and updates to the client's plan of care.
- Monitor client progress toward home health care goals.
- Educate the client and family regarding the plan of care and nursing care needs.
Qualifications :

Minimum Education : Graduate of an Accredited Nursing Program required.
Minimum Experience : 1-2 years home health care experience preferred and 1 year of nursing experience in a health care setting preferred. Strong communication skills. Strong computer skills.
Licenses / Certifications : Colorado LPN license in good standing with the Department of Regulatory Affairs. BLS required. Up to date car insurance and driver license will be required.
