POSITION SUMMARY
The MDS Coordinator is responsible for completing minimum data set assessments and creating comprehensive plans of care after review of the patient’s medical record and communication with direct care staff, the patient’s physician, and family. The MDS Coordinator develops and completes the patient assessment process in accordance with the requirements of federal and state regulations and company policies and procedures.
ESSENTIAL DUTIES & RESPONSIBILITIES GENERAL RESPONSIBILITIES
Participates in prospective residents' admission process in terms of their nursing needs and appropriate placement.
Determines potential Resource Utilization Groups (RUGS) and expense associated with a potential admission.
Reviews all applications for admission, followed by on-site or phone assessment and communicate results of assessment to the Admissions Coordinator.
Complete and assure the accuracy of the MDS process for all residents.
Maintain current working knowledge of Medicare criteria, serving as a resource for nursing staff and communicate changes in regulations.
Monitor Case Mix Index (CMI) scores, looking for potential risks and / or changes that may affect Medicaid Reimbursement.
Facilitate problem-solving for complicated admissions.
Monitor Medicare assessment schedules and nursing documentation to ensure accuracy and timely submission.
Develops care plans and assessment schedules for residents.
Utilizing the RAI, MDS 3.0, QM, CBQM, and MBP manuals to inform activities and assessments.
PREREQUISITES
A bachelor's degree in nursing along with a valid state nursing license.
Medical terminology knowledge.
Previous experience in a clinical and healthcare setting; experience as an MDS coordinator preferred.
An understanding of the MDS 3.0 assessments and procedures.
Knowledge of state and federal regulations.
A solid understanding of data collection, reporting, and analysis.
