We are currently seeking a dedicated RN MDS Coordinator to lead our MDS department. The ideal candidate will play a vital role in coordinating discharge planning and assessing short-term residents. Candidates must have prior experience in MDS (Minimum Data Set) as this expertise is integral to the role.

The RN MDS Coordinator will oversee the gathering of information, assessment of resident needs, and the establishment of attainable goals, culminating in organized, concise care plans. The role involves collaboration with an interdisciplinary team to ensure comprehensive assessments and the incorporation of recommendations into individualized care plans for residents. These plans will focus on addressing all pertinent issues and setting achievable goals for each resident's well-being.

Responsibilities also include ensuring that nursing staff are well-versed in individual care plans, monitoring adherence to these plans, and assessing residents to confirm their needs are being met. The RN MDS Coordinator will conduct regular reviews and revisions of care plans as necessary and schedule and lead weekly care plan meetings for all residents, following OBRA and state requirements.

32 hour positon

Key Qualifications :

• Hold a current state RN license and maintain good standing as a nurse.
• Meet all applicable federal and state licensure requirements.
• Attention to detail, strong follow-through skills, and the ability to prioritize multiple tasks.
• Proficiency in instructing others.
• Thorough knowledge of general, rehabilitative, and restorative nursing practices, procedures, laws, and regulations governing long-term care.
• Experience with Point Click Care is advantageous.
