Job Overview
We are seeking a dedicated and detail-oriented MDS Coordinator to join our healthcare team. The ideal candidate will play a crucial role in managing the Minimum Data Set (MDS) assessments and ensuring compliance with regulatory requirements. This position requires a strong background in medical management, critical care, and health information management, along with excellent communication and organizational skills.
Responsibilities
Conduct comprehensive assessments of residents to complete MDS documentation accurately and timely.
Collaborate with interdisciplinary teams to develop individualized care plans based on assessment data.
Ensure compliance with federal and state regulations regarding MDS submissions and documentation.
Provide training and guidance to nursing staff on MDS processes and documentation requirements.
Monitor resident outcomes and implement quality improvement initiatives as necessary.
Maintain up-to-date knowledge of changes in regulations related to MDS and healthcare documentation.
Requirements
A valid nursing license is required; experience in skilled nursing care is preferred.
Proficiency in PCC or similar electronic health record systems is highly desirable.
Strong background in medical management, including home care experience.
Excellent communication skills, both verbal and written, with the ability to collaborate effectively within a team environment.
Attention to detail and strong organizational skills are essential for managing multiple tasks efficiently.
Previous experience in medical documentation is an asset.
