Oak Street Health is now a part of CVS Healthcare Delivery, and our impact on the communities we serve is large and growing. This Medical Director role will put you at the leading edge of the value-based care movement by being a part of our Central Care Model Development team.This will involve working on projects within Oak Street health and across the broader CVS Healthcare Delivery organization, enabling this role to have even greater impact at scale.
The Medical Director for Population Health will be responsible for co-leading, with Directors and Vice Presidents from the Population Health and Central Services Departments, a range of clinical and strategic initiatives. Population health is a central team within CVS Healthcare Delivery (HCD), and provides strategic leadership, develops technology, and runs central programs used by the various business units such as Oak Street Health, CVS minute clinic, signify health, etc. The data engineering and product teams which designs, builds, and implements technology that support the value based care model is a part of Population Health, and our Medical Directors will be working closely with and providing clinical guidance to that team. The Medical Director - Population Health will provide clinical leadership to a portfolio of programs and planning initiatives within this domain. Examples of potential projects include (but are not limited to) :
- Development of AI technologies to support Value Based Care
- Centralized Telehealth Services
- Liaison with external contracted disease management companies
- Specialty Services embedded in the primary care centers
- Pharmacy and Medication Management activities
Core Responsibilities :
The Medical Director is expected to maintain a current clinical practice in the fields of primary care, internal medicine, or family medicine. This includes clinical experiences within CVS Healthcare Delivery that will inform the Medical Director's leadership and administrative duties. This clinical practice will involve 20-40% of the Medical Director's time. On a case by case basis, applicants wishing to maintain a limited clinical practice outside of CVS Healthcare delivery in addition to core clinical experience within the organization will be considered.
