DescriptionThe Formation Project is a non-profit based in Charleston, SC building a resilient community free from exploitation. The Formation Project serves survivors of sex and / or labor trafficking by providing safety, stability, and connection through case management, peer support, and legal services. The Formation Project increases the identification of survivors and collaborative community partnerships while enhancing the options survivors have available to them regarding justice outcomes.We are seeking a dedicated Case Manager in Charleston, South Carolina, responsible for trauma-informed case management, crisis coordination, and facilitating appropriate referrals to our community partners. This role will directly report to the Program Manager.You will thrive in the role if you are : Able to independently initiate project and initiative ideas; managing projects and initiatives through to completion.Highly creative in generating new ideas and creating novel solutions.Detail-oriented, with the ability to keep track of many small moving parts across a project.Highly flexible and able to gracefully adapt to new changes.Comfortable taking the lead on projects and determining priorities in order to meet deadlines.Salary and Benefits Package : Comprehensive benefits package, including a health reimbursement for employees.PTO accrual at a rate of 176 hours per year.Quarterly mental health days, 9 holidays, and a week-long Winter Break.Potential 1-time moving stipend available.403(b) retirement plan with employer match.ResponsibilitiesKey Duties and Responsibilities
- Provides direct and comprehensive culturally responsive case management, including responding to referrals, conducting intakes and a thorough needs assessment, and collaborating with the survivor to prioritize and meet service needs that may include safety planning, basic necessities, access to crime victim benefits, language access, housing, health, and medical, education and employment, among others.
- Responds to crisis calls from clients during work hours.
- Data entry and case notes per TFP protocols.
- Identifies key resource gaps and reports these to the Program Manager and Community Coordinator.
- Provides support to participants through the program structure and goals in a timely and sustainable manner.
- Works to strengthen partnerships and community collaborations.
- Serves as an advocate for survivors within systems that often create barriers for survivors of crime ensuring that survivors have access to crime victim benefits.
- Participates in professional development as required and / or agreed upon by supervisor.
- Record and submit detailed documentation in a timely, thorough, and accurate manner.
- Provide ongoing feedback to your supervisor and the CEO on how we can best serve the mission.
- Performs other duties and assumes other responsibilities as assigned by supervisor.Schedule :
- The typical working hours are Monday - Friday, 40 hours a week, between the hours of 8 : 00 am - 6 : 00 pm.
- Work is in person, with some remote options depending on the supervisor's request and the client's needs.
- Periodic on-call may be necessary due to the crisis nature of this work.QualificationsRequired Experience and Skills
- Experience or education in social work or human services.
- Fierce care for empowering others and a strong passion for access to justice.
- An understanding of the provision of trauma-informed services.
- A team-oriented attitude.
- Knowledge of Microsoft Office (including Excel, Word, and PowerPoint), Adobe Acrobat Pro, and Zoom.
- Comfortable working in a dynamic, growing environment.Preferred Experience and Skills :
