Ohioans Home Health & Hospice is seeking an experienced and compassionate Hospice Nurse Liaison to focus on evaluating current home health patients and transitioning those who may benefit from hospice care. This role combines clinical expertise, education, and compassionate communication to guide patients, families, and care teams through this important decision.
Key Responsibilities :
Patient Assessment : Evaluate current home health patients to determine hospice eligibility based on clinical criteria and disease progression.
Education & Advocacy : Provide disease process education to patients, families, and caregivers, helping them understand prognosis, symptom management, and the benefits of hospice care.
Smooth Transitions : Collaborate with home health teams to facilitate seamless transitions to hospice care, ensuring patients and families feel informed and supported.
Clinical Collaboration : Partner with physicians, home health staff, and other healthcare providers to identify patients who may benefit from hospice services.
Relationship Building : Serve as a trusted resource and liaison between patients, families, and healthcare providers to ensure open communication and trust.
Community Engagement : Actively promote the benefits of hospice care within the organization and with external referral sources to increase awareness and understanding.

We are looking for candidates with :
Current RN license in the state of Ohio
Minimum of 3 years of nursing experience, preferably in hospice, home health, or palliative care.
Strong knowledge of disease processes, symptom management, and hospice eligibility criteria.
Exceptional communication, interpersonal, and teaching skills, with the ability to explain complex medical information in a compassionate and clear manner.
Highly organized, proactive, and empathetic in patient interactions.
Experience in healthcare marketing, outreach, or patient advocacy is a plus.
