We are seeking a compassionate and experienced Inpatient Social Worker to join our behavioral health team within a specialized Acute Psychiatric Unit . This role plays a vital part in supporting patients through psycho-social assessments, individualized care planning, therapeutic interventions, and safe, coordinated discharge planning.
You'll work as part of a dedicated interdisciplinary team, contributing to the delivery of high-quality mental health care in a supportive, structured inpatient setting—primarily serving a geriatric psychiatric population
Key Responsibilities
- Complete timely psycho-social assessments and progress notes for all patients, incorporating input from families and support systems
- Collaborate with medical and clinical staff to develop, implement, and update individualized treatment plans
- Initiate and manage discharge planning from the time of admission, ensuring smooth transitions to appropriate levels of aftercare
- Lead 1–2 therapeutic groups weekly, providing support and structure to patients within a safe group setting
- Participate in interdisciplinary rounds, policy development, and quality improvement initiatives within the unit
- Maintain accurate and timely documentation for all services in accordance with clinical and regulatory standards
- Help coordinate involuntary transfer processes when required and support elopement prevention procedures
