SummaryPosition : Clinical Social WorkerReports to : Manager of Social ServicesSummary : Provides short-term counseling, crisis intervention, and discharge planning services to diverse patient populations. Completes comprehensive psychosocial assessments to determine necessary clinical interventions and assess discharge planning needs. Actively coordinates continuity of care planning and collaborates with a multidisciplinary team to develop individualized treatment plans. - Conducts comprehensive psychosocial assessments and histories to determine treatment and discharge planning needs.
- Interviews patients and / or families to gather medical, financial, social, and psychological data.
- Reviews patient progress and assesses the ongoing level of care.
- Collaborates with medical, nursing, and multidisciplinary staff for seamless coordination of care.
- Engages patients and families in care planning and treatment decisions.
- Ensures thorough and accurate documentation of patient care, discharge planning, and clinical interventions.
- Identifies patients and families eligible for financial assistance programs and refers them appropriately.Clinical Therapy Services :
- Provides short-term therapeutic counseling for patients, families, and significant others.
- Conducts daily group therapy sessions tailored to patient needs.
- Assists patients and families in understanding the psychosocial implications of medical diagnoses.
- Facilitates interdisciplinary meetings to develop and coordinate adequate treatment plans.
- Works with Utilization Review and other departments to monitor and support patient clinical status and disposition.
- Identifies and resolves barriers to effective patient care.Discharge Planning & Continuity of Care :
- Identifies patient and family service needs, referring them to community agencies and resources.
- Develops continuity of care plans, ensuring smooth transitions post-discharge.
- Acts as a patient advocate and mediator in conflict resolution.
- Coordinates with outside providers to ensure successful aftercare transitions.Community Engagement & Professional Development :
- Provides consultation and education to agencies and community groups.
- Supervises MSW student interns.
- Builds and maintains collaborative relationships with community providers and agencies.
- Stays current on developments in the field through continued education, professional affiliations, and literature reviews.
- Participates in hospital activities and committees as needed.Organizational Responsibilities :
- Attends department meetings and stays informed.
- Completes mandatory education, competencies, and health requirements.
- Maintains licensure and certifications as required.
- Adheres to universal precautions and safety standards.
- Complies with hospital policies and regulatory requirements.
- Available for overtime and flexible shifts when required.

- Master's degree in Social Work (MSW) required.
- Knowledge of psychiatric disorders, treatment modalities, and case management.
- Minimum two years of experience in hospital or healthcare social services preferred.
- Licensure or eligibility for licensure as a Licensed Clinical Social Worker (LCSW) in Indiana required within three years of obtaining necessary clinical hours.Knowledge & Skills :
- Proficiency in clinical assessment, problem-solving, and counseling.
- Familiarity with community resources, financial assistance programs, and protective services.
- Understanding of medical insurance, managed care, and discharge planning.
- Strong communication, organizational, and crisis management skills.
- Cultural competency and ability to work with diverse populations.
- Proficiency in computer applications and electronic documentation.
- Ethical, professional conduct with discretion and confidentiality.Working Conditions & Physical Demands :
