The Position :
St. Patrick Center, a ministry of Catholic Charities, is hiring a part-time Shallow Subsidy Case Manager to join our Supportive Services for Veteran Families (SSVF) team. This case manager will utilize wrap-around philosophy and techniques to guide veteran families on their journey to self-sufficiency in the community. We are looking for a passionate case manager to assess client strengths, create strategies to achieve goals, and coach clients in learning skills for problem-solving. The case manager will coordinate individualized services, housing placement, and community involvement for veteran families. The core working hours are 8 : 00 am to 4 : 30 pm, Monday through Friday.
