The Nurse Case Manager will provide help to high-risk infants and children getting health care and developmental services. The Nurse case manager will complete the following :
- promote optimal growth and development of infants.
- provide emotional support and relevant referrals to families for essential services.
- work with the mother to reduce stress and help explain identified medical conditions for further evaluation, diagnosis and treatment.
- responsible for maintaining a caseload of approximately 100 active clients by conducting outreach activities and utilizing the referral system from High-Risk Infant follow Up Program from the State of Illinois.
