We are seeking a compassionate and experienced Registered Nurse to join our Home Health Care team. As a Home Health Care Registered Nurse, you will provide high-quality, patient-centered care to patients in their homes, focusing on improving health outcomes, reducing hospital readmissions, and enhancing patient satisfaction.

• Responsibilities :
• Conduct initial and ongoing assessments of patients' physical, emotional, and social needs.
• Develop and implement individualized care plans in collaboration with patients, families, and healthcare providers.
• Provide skilled nursing care, including wound care, medication management, and education.
• Monitor patients' progress, identify potential complications, and take prompt action to address them.
• Educate patients and families on disease management, treatment options, and self-care techniques.
• Coordinate care with other healthcare professionals, including physicians, therapists, and social workers.
• Document patient information accurately and timely, maintaining confidentiality and HIPAA compliance.
• Participate in case conferences, quality improvement initiatives, and team meetings.
• Stay up-to-date with industry developments, best practices, and regulatory requirements.
• Collaborate with the interdisciplinary team to achieve patient goals and optimize outcomes.
• Requirements :
• Current RN licensure in the state of practice.
• Minimum 1-2 years of experience in home health care, hospice, or acute care.
• Strong clinical skills, critical thinking, and problem-solving abilities.
• Excellent communication, interpersonal, and patient education skills.
• Ability to work independently and as part of a team..
• Familiarity with electronic medical records (EMRs) and mobile documentation systems.
• Reliable transportation and ability to travel to patient homes.
• Preferred Qualifications :
• Home health care experience or certification (e.g., HHC RN, OASIS).
• Knowledge of Medicare regulations and guidelines.
• Experience with wound care, chronic disease management, or palliative care.
