Home Health Community Liaison / Marketer (salary and monthly bonus)

We are looking to add a Community Liaison / Marketer to our team.

Responsibilities for this role include :

  • Coordinating referrals for home health care services
  • Establishing new contacts with referral sources within the community
  • Strengthening relationships with past referral sources
  • Providing updates on referrals at weekly marketing meetings

The ideal candidate in this role needs to be self-directed and able to work with minimal supervision. Prior home health marketing / sales experience is required. Strong communications, presentation and interpersonal skills are essential.

The territory is Boulder, Longmont, Louisville, Lafayette, Broomfield and the surrounding areas. Applicants must reside in or close by this area and have home health contacts within this territory.

Note : starting pay is $60,000-$80,000 annually (dependent upon experience) and there is an additional uncapped monthly bonus based on referrals.

    We are looking to add a Home Health Aide (CNA). CNAs provide direct patient care in a patient’s home according to a patient’s specific plan of care prepared by the RN / Case Manager.

    CNAs have the ability to choose their workload and collaborate to find a schedule that works around other commitments.

    Nursing students are encouraged to apply as this position counts toward patient contact hours for most programs.

    Patients will typically be located 30 minutes or 30 miles from the aide’s home.

    The Aide / CNA must :

  • Have a high school diploma, GED or equivalent combination of education and / or experience
  • Be currently licensed as a CNA in Colorado
  • Responsibilities include :

  • Providing aide services to patients as identified in the plan of care per agency policies, procedures and aide skill level.
  • Providing total or assisted personal hygiene per plan of care.
  • Providing patient / caregiver assistance in ambulation, exercises, transfers and medication per plan of care.
  • Assisting in maintaining a clean, safe patient environment and may perform homemaker chores as assigned in the plan of care and as skill level permits.
  • Assisting with patient personal health self-care and comfort measures.
  • Observation, reporting and documentation of patient status and the care or service furnished.
  • Pay rate : $19-$22 / hour (dependent upon experience)

    Note : Regular weekday availability is needed. Visits may be 1-2 hours typically. This is not shift work and there are no overnight hours.

    This position travels to adult and pediatric patients homes and assists with hands on therapy to expedite healing and recovery.

    This position has a great deal of flexibility and allow you to create your own schedule.

    Prior pediatric experience and / or comfort working with pediatric patients preferred. Will train the right candidate.

    Applicants should have multiple weekdays of regular daytime availability in their schedule to see at least 3-5 patients / complete 10-15 visits per week.

    Patients will typically be located 30 minutes or 30 miles from the OT’s home.

    Responsibilities include :

  • Performs comprehensive initial and on-going occupational therapy assessments per standards of practice on each assigned agency patient and develops the plan of care in collaboration with patient, RN Case Manager, and physician.
  • Regularly evaluates the effectiveness of each assigned patient’s plan of care, response to treatment, revision needs, discharge plans, and progress to patient goals.
  • Establishes goals to enhance the rehabilitative ability of the patient, which may include but not be limited to : neuromuscular facilitation / inhibition; perceptual, prosthetic, and kinetic treatment; orthotics / splints; activity of daily living training; and related to needs.
  • Evaluates the safety of the home environment and recommends to patient / representative potential solutions for identified concerns.
  • Pay rates : Visit rates : $70-$125 / visit (based on type of visit and dependent upon experience) Hourly rate for meetings, case conference, training, orientation : $35-$45

    The Physical Therapist will be partnering with the primary care physician to create a care plan that reflects the patient’s needs and physician’s goals for recovery. Take advantage of the ability to make your own schedule and make a true difference in the lives of those for which you’re caring.

    Prior pediatric experience and / or comfort working with pediatric patients preferred. Will train the right candidate.

    Applicants should have multiple weekdays of regular daytime availability in their schedule to see at least 3-5 patients / complete 10-15 visits per week.

    Additionally we need someone to do in-person and telehealth PT visits.

    Patients will typically be located 30 minutes or 30 miles from the PT’s home.

    Responsibilities :

  • Performs comprehensive initial and on-going physical therapy services for each assigned agency patient and develops the plan of care in collaboration with patient, RN / Case Manager, and physician.
  • Regularly evaluates and documents the effectiveness of each assigned patient’s plan of care, patient responses, revision needs, discharge plans, and progress to patient goals.
  • Based on patient needs, seeks physician orders for equipment to promote patient comfort and safety, and when ordered and received by patient, initiates equipment use training.
  • Evaluates the safety of the home environment and recommends to patient / representative potential solutions for identified concerns.
  • Establishes therapy goals to enhance the rehabilitative ability of the patient, which may include but not be limited to : hot packs, gait training, diathermy treatments, therapeutic exercises, passive and active range of motion exercises, whirlpool baths and other related treatments to meet the agency patient’s needs.
  • Meets productivity requirements established for home health care physical therapist.
  • Supervises assigned PTAs
  • Pay rates : Visits : $65-$125 / visit (based on type of visit and dependent upon experience) Hourly rate for meetings, case conference, training, orientation : $35-$45
