As a Nurse Care Manager, you will play a pivotal role in identifying, screening, planning, tracking, and coordinating personalized care plans for our consumers.Responsibilities : Collaborates with consumers to assess their conditions and needs, developing, and supervising personalized care plans.Conducts comprehensive nursing assessments to gather physical and behavioral health information.Advocates for consumers and their families, linking them to resources and empowering them to improve their overall health and wellness.Utilizes motivational interviewing skills to empower consumers towards better health outcomes.Leads interdisciplinary treatment team meetings to review assessments, updates care plans, and determines follow-up frequency.Maintains positive working relationships with primary care providers and social service agencies, collaborating to obtain resources and meet consumer needs.Complieswith agency policies and procedures, ensuring documentation deadlines and productivity expectations are met.
