Position Overview

The Nurse Practitioner (NP) reports to the Medical Director, with accountability for providing strategy, judgment, organization, and evidenced-based analysis to influence decisions, and directly to meet Care at Home's requirements.

Key Responsibilities

The NP will have the following responsibilities :

• In partnership with the clinical team, provide medical highest quality medical care for the practice’s patients.
• In partnership with the Medical Director and Management, assist and support clinical personnel (e.g., clinical nurses, allied health professionals, EMR, telemedicine operations).
• Provide consultations and perform diagnostic and therapeutic procedures in their clinical discipline within their practice scope, consulting when appropriate the supervising physicians for assistance with both diagnosis and treatment.
• Works collaboratively with a multi-disciplinary team to create individualized treatment plans for patients; embrace and promote a patient-centric model of care.
• Commit to the practice of value-based care which emphasizes care in the appropriate setting to improve and lower costs, address and close gaps in care and drive high patient satisfaction.
• Demonstrate proficiency and support use of the practice management system and electronic medical record (EMR).
• Perform other job-related duties as assigned.
