We're currently partnered with a growing Home Health organization looking for RNs in the Santa Cruz County area.

This role is pay-per-visit which directly allows for higher earning potential and more autonomy. This role offers PPV rates $90-95 / point (plus mileage) with an admin rate of $65 / hr.

Key Responsibilities :

• Maintain 30 points per week.
• Conduct comprehensive assessments of patients' health status and develop individualized care plans.
• Administer medications, treatments, and therapies as prescribed by the physician.
• Monitor patients' conditions and report any changes or concerns
• Educate patients and their families on disease management, medication adherence, and self-care techniques.
• Collaborate with other healthcare professionals, including therapists, social workers, and home health aides, to ensure holistic care delivery.
• Maintain accurate and timely documentation of patient assessments, interventions, and outcomes in electronic health records.
