Being a Nurse Navigator RN provides a unique opportunity to work onsite in a collaborative environment, coordinating care and guiding patients through their health journeys without direct bedside care. WHAT YOU WILL DO :

• Be a Catalyst for Change : Use your nursing expertise to connect mobile healthcare resources, integrating services and innovations to improve patient outcomes.
• Telephonic Patient Assessments : Assess patient needs remotely using evidence-based protocols, helping them navigate the complex healthcare system.
• Empower & Educate : Provide vital health education, support healthy behaviors, and help patients understand their care options across the healthcare continuum.
• Leverage Technology : Engage in telemonitoring activities, utilizing cutting-edge tools to track progress and intervene proactively.
• Lead & Collaborate : Play a key role in team care planning, mentoring colleagues, and championing new approaches in healthcare delivery.
• Promote Our Mission : Represent GMR's innovative programs within the community, reaching populations that need care most but often go underserved.
• Facilitate & Educate : Serve as a facilitator and educator for GMR's integrated healthcare training, empowering others to deliver better care.
• Deliver Excellence : Manage your time, resources, and documentation efficiently while providing outstanding customer service and support.
