Job Summary :
We are looking for a dedicated and experienced Director of Nursing to lead our home health services. The Director of Nursing is responsible for supervising home health care to homebound patients in their place of residence, ensuring that services are provided according to physician orders and plans of care. This leadership role is focused on providing the highest quality of care and ensuring the successful coordination of clinical services. If the Director is an RN, they will directly oversee skilled nursing and other therapeutic services, with availability during operating hours (or appoint a qualified alternate).
Responsibilities :
- Develop and enforce standards for safe, high-quality care that meets patient and family needs.
- Collaborate with the Administrator to set objectives and ensure their successful implementation.
- Determine staffing needs to meet organizational goals, ensuring efficient utilization of health personnel.
- Oversee the recruitment, hiring, evaluation, and termination of service personnel.
- Facilitate staff orientation, training, and continuing education.
- Maintain up-to-date policies and procedures to ensure compliance with regulatory requirements.
- Provide ongoing support and guidance to staff, addressing job-related stress and professional development.
- Ensure ongoing assessment of patient care needs and the interdisciplinary team’s plan of care.
- Act as a liaison between the agency and other community services to enhance patient care.
- Serve as a role model in dress, conduct, cooperation, and job performance.
- Maintain patient confidentiality and adhere to ethical standards.
- Ensure all patients receive timely and effective care through on-call availability (24 / 7).
- Oversee the Agency’s Quality Assessment and Performance Improvement (QAPI) program.
- Manage patient care plans and ensure ongoing assessments of patient / family needs.
- Participate in community education efforts and promote agency visibility in the local area.
- Ensure all necessary equipment and supplies are available for patient care.
- Address any patient care concerns and resolve issues promptly.
