Job descriptionPerform comprehensive OT evaluations in patients' homes Develop and implement individualized treatment plans Provide skilled interventions to improve function, mobility, and safety Educate patients and caregivers on exercises, safety, and injury prevention Monitor progress and adjust care plans as needed Document all visits accurately and timely in the EMR Collaborate with physicians, nurses, and care team members Maintain compliance with Medicare, HIPAA, and agency standards Graduate of an accredited OT program Active Pennsylvania OT license Minimum 3 years of clinical experience (home health preferred) Strong assessment, clinical reasoning, and documentation skills EMR experience preferred (Kinnser / WellSky or Homecare Homebase) Reliable transportation and valid driver's license COTA supervision experience preferred
