POSITION OVERVIEW Ain Dah Yung Center is looking for a full time The Street Outreach Case Manager who will work in conjunction with the collaborative- "StreetWorks," through conducting Street-based outreach to homeless youth. Focus will be on building strong networks to provide longer term Case Management to youth with a focus on Shelter, housing, and cultural connections.ESSENTIAL FUNCTIONS This position will be responsible for coordinated entryStreet Outreach Case Manager will be required to attend all Agency and Street Outreach meetings / trainingsStreet outreach shifts will include some late nights and weekendsHMIS data entry for Outreach and Case Management clients RequirementsQUALIFICATIONSBachelor's degree in social work, human services, child development, elementary / secondary education or related field preferred.
