About this role :
Under the direction of the Program Coordinator, the Family Partner collaborates and provides supportive services to assigned families. Planning and implementing the identified services needed to assist families in the development of skills to increase parenting abilities, including behavior management training.
Schedule : Tuesday through Saturday, 10 am – 6 pm
In this role, you will get to :
- Collaborate with participants and families on a weekly basis
- Establish and maintain relationships with other agencies and organizations in the community including DTA, SS, and WIC
- Ensure compliance and adherence to the Kennedy-Donovan Center and the Department of Children and Families philosophy and mission statements
- Assess family needs while developing safe and helping relationships with assigned individuals
- Participate in the development and adherence of treatment plans
- Provide advocacy for families as needed
- Collaborate with internal / external resources and community providers for placement services
- Assist with pediatric medical support and childcare needs
- Participate in team meetings, including DCF treatment plan utilization reviews, IEPs as needed, Foster Care Reviews and court trials as needed
- Comply with all KDC and DCF documentation requirements.
- Provide life organizational assistance, job placement, mental health, and substance abuse resources.
- Provide transportation as needed.
