The Primary Care Outpatient Staff Registered Nurse (RN) is responsible and accountable for individual nursing practice and determines the appropriate delegation of tasks consistent with the nurse's obligation to provide optimal specialty care. This nurse provides patient care, care coordination, health promotion, health education and coaching on wellness, disease prevention, and chronic care management. The RN is responsible for providing competent, evidence-based procedural care to assigned patients with oversight of licensed vocational nurses or health / nursing assistants as appropriate to the procedural setting. Assesses the patients physical and mental status prior to procedures, educates on procedural and postprocedural care, and documents responses to interventions, findings, and reports any changes. Partners with the patient, family, and interdisciplinary team to promote effective and safe transitions across the health care continuum, including the Veteran's goals of care. Ability to formulate a plan of care based on nursing assessments and Veteran goals of care. Seeks guidance on how to improve care provided to patients and families. Delegates elements of care to the appropriate individual based upon assessed needs, condition, and safety of the patient. The nurse brings principles and theory with limited application of critical thinking skills. The Primary RN may function in several roles to include, pre-procedure assessments, post procedural recovery, and assisting physicians during procedures. They maintain proficiency on the maintenance, cleaning, and preparation of procedural specific equipment. They recognize changes in patient care situations and initiates appropriate emergency situations. VA Nurse Total Rewards
