Job DetailsJob LocationCHI - Modesto - Spyres - Modesto, CASalary Range$104,000.00 - $139,776.00 SalaryJob ShiftAnyDescriptionSUMMARY : The PCM is a registered nurse, duly licensed in California, who is responsible for the ongoing management of the patient care team to ensure the delivery of quality patient care, as called for in the Plan of Care (POC) adopted by the Interdisciplinary Team.RESPONSIBILITIES : Manages patient care team staffing on a 24-hour basis, including on-call and weekend coverage to meet changing organizational needs and assure quality patient / family care. Reviews and conducts performance appraisals of staff on a regular basis. Standard : Educates, evaluates, counsels, and terminates nursing personnel following current Community Hospice policies. Consults with CCO and / or Director of HR regarding all disciplinary and performance evaluations. Manages staff to meet patient census needs, including full time, part time, per diem, and night staff, through staff scheduling, supervisory visits and review of clinical documentation.Is responsible for ensuring that hospice services, documentation of those services, and clinical care is provided according to professional standards and in compliance with all current regulations and conditions of participation.Standard : Reviews chart audit results routinely to assess team members' education and support needs and counsel as needed. Schedules supervisory visits with personnel not less than annually. Assists with focused auditing of patient records as assigned by CCO. Consults on a daily basis with team members to guide care and educate staff. Provides liaison / oversight to skilled nursing facilities (SNF) and Residential Care Facilities for the Elderly (RCFE), as assigned. Participates in patient care conferences at SNF / RCFE and reviews all quality assurance reports with SNF Director of Nursing or designee and / or the RCFE Administrator to resolve patient care issues, staff communication, and coordination of services per Medicare / Medi-Cal, and Community Care Licensing regulations.3. Reviews and revises policies and procedures manuals, relevant to patient care issues, on an ongoing basis as required to meet changing needs and to assure compliance with current regulations.Standard : All departmental policies and procedures are reviewed per policy with the appropriate Interdisciplinary Team core group members, senior management, and the Board of Directors.4. Participates in the recruitment, hiring, retention, orientation, and ongoing training of nursing personnel; this may include personnel who are orienting to other areas of Community Hospice, as well as those who will be assigned to case management.Standard : Works with the CCO, other PCMs, and the Human Resources Department to carry out effective strategies of employee recruitment, selection, hiring, orientation, retention, and staff training. 5. Facilitates communication between the Interdisciplinary Team and demonstrates good problem-solving skills while utilizing a team-based approach.Standard : Presides over Interdisciplinary Team meeting, assuring that all problems have been addressed in the POC as appropriate. Facilitates the flow of information, regarding patient status, between all agency departments.6. Coordinates communication and services with contracted companies, (e.g., Durable Medical Equipment [DME], infusion services, pharmacy, etc.) to ensure that patient care and patient / family teaching standards are met.Standard : Communicates with DME, Infusion services, pharmacy providers regarding appropriate patient teaching, equipment setup, and safety setup, and safety standards. Reviews and resolves all quality assurance incidents related to service provider contracts.7. Provides administrative on-call function on a rotational basis.Standard : Will be available by phone / pager during non-office hours to provide administrative / clinical support to Community Hospice night or weekend staff.8. Participates in Performance Improvement Strategies Committee and other committees as assigned.Standard : Participates in identifying issues of concern and / or quality improvement issues to the Quality Assurance and Performance Improvement (QAPI) Committee as appropriate. Provides any pertinent records / statistics as may be required. Attends QAPI meetings on a regular basis.9. Responsible for maintaining a safe working environment for all hospice staff.Standard : Observes all safety requirements as outlined in the Injury and Illness section of the Policy and Procedure Manual. Assures that there are no instances of unsafe work practices by nursing staff, i.e., failure to adhere to universal precautions.10. Performs job duties in a professional manner and maintains confidentiality of patient information at all times.Standard : Maintains a professional appearance of self and working area, exhibits appropriate behavior with patients and families : i.e., respectful, courteous and professional at all times. Listens to staff, encourages discussions, and participation. Maintains good working relationships with community physicians and referring organizations.11. Must maintain RN license in accordance with federal, state and local laws, and must act only within the scope of his or her state license. All personnel qualifications must be kept current at all times.Standard : 100% Compliance12. Participates and provides in-services, workshops, and clinical meetings to promote professional growth and understanding of hospice care for both CHI staff as well as community partners.Standard : Attendance at 80% of all mandatory in-services and meetings, designated as appropriate, provided by agency. Completes all required in-services. 13. Follows, enforces and monitors staff compliance with all Community Hospice policies and procedures including, but not limited to, safety, infection control, privacy and confidentiality, and HIPAA.Standard : 100% Compliance14. Fosters relationships and communication with facility management to promote coordinated, comprehensive continuum of quality of care to hospice patients. Standard : 100% ComplianceOther Functions : Participates in committees as requested by the Board of Directors, President / Chief Executive Officer / Administrator and / or the CCO.Administrative duties as requested by the Board of Directors, President / Chief Executive Officer / Administrator and / or the CCO.Provides education, utilizing a number of different media, regarding hospice services to patients, families, staff, and the community as needed.Responsible for establishing and maintaining a safe environment for patients / families, and a safe working environment for Community Hospice staff.Performs job duties and interacts with patients / families, Community Hospice staff, and community members in a courteous and professional manner. Maintains all required confidentiality and privacy requirements of both the organization and the regulatory agencies that have oversight of and for Community Hospice.The job demands that the PCM also be able to perform all of the routine duties of a Registered Nurse in a home-like environment (including those living in facilities) when needed.Performs all duties as assigned by the CCO, including work in other areas, to cover absences or relief to equalize peak work periods or otherwise meet the needs of the organization.SUCCESS FACTORSThe PCM must continuously exercise a strong commitment to the organizational goals of Community Hospice while simultaneously exhibiting intelligence, humor, and sensitivity to extremes of human need. Meet deadlines and remain on timelines.Routinely work without supervision from the CCO.QualificationsQUALIFICATIONS : Current California license to practice as a registered nurse. BSN status is preferred.One year of acute medical / surgical experience, skilled facility experience or equivalent preferred.Current CPR at the Basic Cardiac Life Support (BCLS) level.Current home health, hospice, or community health experience of two years is preferred. Certification as a Hospice and Palliative Nurse is preferred.One year of home health, hospice, or SNF management experience is preferred.Computer knowledge and experience working with electronic patient records, as well as various other applications.Recommended minimum WPM of 23.Must be able to maintain professional attitude, while managing multiple tasks, in a faced paced, emotional environment.
